                                 Age as of 11/30/12. 14 year old can not be in 9^th grade

                   yours to keep at end of season!

Ø  Fees:  Registration Fee + Raffle + Work Bond

                   Registration Fee - 1^st Child Football $155, 2^nd Child Football $130

                                   1^st Child Cheerleading $130, 2^nd Child Cheerleading $120

                          Higher Amount will be used 1^st - Maximum cost 2 Children Plus additional raffle per child

Raffle $60 (per child)                       

Work Bond $100 (Per Family)

               Refundable or roll over option after Season, if approved Volunteer hours are completed

           by PBYFL, as we need to use the multiple part paper

     The form must be dated after January 1^st 2012, have Doctor’s stamp, signature and the

     words ‘Able to participate in the 2012 season’.

 Parents Code of Conduct Form

                   We are looking for signatures from all Parents or Guardians who will attend Practice or Games
